Ascend Community Wellness was founded in December 2025 with the goal of offering care that truly meets people where they are. We know that in larger agencies it’s easy for clients to feel overlooked or like their unique needs aren’t fully understood.

Our approach is different. We focus on individualized, person-centered support that is meaningful and responsive to each person’s experiences and goals. Our mission is to provide this level of care while creating a sense of community where clients feel seen, heard, and supported. We are committed to helping people grow, build independence, and improve their well-being through therapy, community integration, and other services tailored to their needs.
